Default formal phone call - urgent, please!

I have to make a formal (business) phone call. How do I ask politely who am I talking to (I don't remember the person's name)?

Is it someone you've met before? In that case, you could say 'Sorry, I've forgotten your name. Who is it I'm talking to again?' Or - depending on the level of formality (this is pretty colloquial) - instead of the first sentence you could say 'Sorry, I've got a memory like a sieve.'
