I think it is to speak to someone direct (with the face)
It can mean "face to face", but it also means without feint; to get straight to the point/matter/subject. "Mincing words" is another similar expression.
"...without mincing words" means to approach the topic head on, often without such nice formalities as euphemism, diplomacy, or even tact, if the situation warrants it. You might approach someone head on, or you might approach the matter head on. It's usually suggests confrontation, but it can sometimes just mean there is no time to waste.
