Hi all,

In the HBO series Six Feet Under,
season 2 episode 5 - "The Invisible Woman".
Nate and David is taking about what a dead woman named Previn should wear.
Nate takes out two dressings, blue and pink, to let David pick.
Here's their conversation:

Nate:
Which one do you think will look better on Miss Previn?

David:
The pink one.
Didn't she specify which gown she wanted, or did Dad slip up?

Nate:
No, Dad didn't slip up.
She wanted to be buried in her own clothes.
The outfit she came in with was shot.

David:
Give her the blue to get rid of it.
It's coming apart in the back.

The phases and sentences which I marked with blue are my questions.
First, does "slip up" mean "make a mistake"?
Second, what does Nate mean when he said her outfit was "shot"?
Third, David wants to get rid of what?
Finally, what does "it's coming apart in the back" mean?

1, Slip up does mean make a mistake
2, in this context shot means ruined beyond repair
3, David wants to get rid of the blue dress
4, coming apart at the back in this context means that the dress is torn at the back.
