Hello,

could you please tell me how you call the account to which your employer pays your wage? Is it "wage payment account"?

And is it possible to use it as follows:

Statement of the employee of his wage payment account - meaning a declaration of the employee concerning the number of his account to which he wants his employer to transfer his wage.

I would call it Employee Bank Details for Salaries & Wages

In the US, the form that the employee would fill out is a Direct Deposit Authorization Form. This form gives his employer permission to deposit his wages directly into whatever bank account he designates.
