I have just prepared a report and going to fax it to Head Office. Is the following sentence correct?

Note: This report is prepared based on the invoices received from Head Office, Bombay, till date.

is prepared "or" was prepared. If is prepared is correct, then When we use was prepared?

You could say is being prepared if the report were still in preparation. However, since it is finished, say was prepared.
