Angry Addressing a person in a formal letter

Is it incorrect to address a person using the following format in a formal letteR, i.e. can BOTH the first and last name be used after Ms. / Mr.?
"Dear Ms. Jane Doe"
or
"Dear Mr. John Doe"

Thank you!

You use only the family name.
'Dear Mr/Ms Doe'
or
'Dear Mr and Ms Doe'
