Could sone one tell me whether the below mentioned statement is correct ?

My phone has gone bad.

It may well be correct - but perhaps it would be better to say "My phone has gone wrong" or "My phone has stopped working"

One common way of sayiing that, particularly if you are reporting to the phone company is "My phone is dead." Just as one would say, "the batteries are dead."

Maybe, something is wrong with my phone or my phone has broken down.
