Cool do/does/did

Why do you sometimes use the auxiliary (do/does/did) and sometimes you don't?
Ex: "What did you eat for lunch?"
"What caused the accident?"

because, in your first sentence, you have a subject( you)

but in the second one, there is no subject, so there is no auxiliary verb
