| Dear Mr/Ms _____________ I am writing to inform you that I will be taking __________ weeks maternity leave beginning on ___________(start date of leave). I plan to return on ___________ if all goes well and my physician is agreeable. Sincerely, You should find out what your company's requirements are as far as requesting maternity leave - do you need to contact only your supervisor, or the Human Resources Department, etc? |
