Editor- Have you submitted a book or article to a publishing house?
To whom is this letter written if not an editor?
Last period- possibly last semester? (what period of time is this?)
What type of paper, exam, thesis, essay?
Four months ago an item was graded that you wrote, and you responded in writing addressing this person's comments?

K-hesham71- This is on the formal side, which I think will serve you better than an informal inquiry.
I am still unsure re. reviewing period ( of the article your submitted? ).
So, you can tailor this format to suit your needs.
