John Test - speaking assessment

Does anyone have an electronic copy of the John Test? I am looking for an assessment to evaluate speaking for our private tutoring business customers, and I have heard that the John Test is used some places, but I have not been able to find a copy of it or even a detailed description. If you have it or if you know the questions and basic pictures that are involved so I could create my own, could you please let me know? Or if you have another recommendation, I am open to that too.
