The way I'd see the sentence is as follows:

On a Valentine's Day, I often make a special gift to my girlfriend by myself, and then I often make a surprise before giving her the present.

I'm not a native nor a teacher

But what about ....."spring a surprise"...rather than "create a surprise"

Does it sound more appropriate?, I don't know for sure, correct me if I'm wrong.

And, is it "On a Valentine's Day" or "On Valentine's Day" ? I would say it's the latter.
