Default fell prone, tripped

I fell prone after I tripped on the stone, grazing my toes.

I meant by the sentence that `I fell flat after I caught my foot to the stone and lost my balance, grazing my toes']

Is this sentence correct?

Please help me.

Say:
I tripped on a stone and fell.
Or:
I tripped on a stone and fell down.
Or:
I tripped on a stone and fell flat on my face.

In the US, you'd say you "stubbed your toe."

I stubbed my toes on the rock and ended up falling flat on the ground.

Which is more likely--that you stubbed your toe and then fell or that you tripped and then fell?
