What is considered a "formal name" when addressing envelopes, invitations, etc.?

Use the addressee's full given name and any appropriate titles, not a nickname:
Ms. Margaret Smith instead of "Peggy Smith"
Mr. Charles Winchester III instead of "Chuck Winchester"
