In "We have a measured rate service for local calls. It gives your local calls for a low monthly rate plus a rate of per call and per minute.",
does "a rate of per call and per minute" mean "how much it costs per call and how much it costs per minute",
or "how many times you make a phone call and how long you talk on the phone in total"?

This is a bit strange. It does mean how much it costs per call and how much it costs per minute". Maybe there is a minium charge per call. If not, I don't see why you have the two together. ;-(

'Cause the Chinese translation for it says it means the telephone fee = monthly rate + how many times you call + how long you talk on the phone.
This makes me confused about the meaning of "rate".
And also...it doesn't sound very logical...

Rate per call means, how much it costs per call and how many times you make a phone call.
Rate per minute means, how much it costs per minute and how long you talk on the phone during that call.
The company is charging its customers for a) each call and b) the time each call takes. So, let's say I make 5 calls, and each call is about 2 minutes in length. The company will send me the following bill:
Number of calls made: 5 calls x FEE = $__.__
Total minutes of calls: 2 minutes x 5 calls = 10 minutes x FEE = $__.__
Total Charge: Calls $__.__ + Minutes $__.__ = $__.___

I guess it sounds like a rip-off to me as well.

A rip-off is an unfair exploitation of someone, especially when they don't have much choice. It seems unfair to charge twice for the same call.
