decided
1-I decided to leave again.
Could this mean that it was the second time I was making the decision to leave, not that I decided to leave after having left and come back once?

Here you may take both meanings into considreation, it depends on the tone of the sentence, I think.

It could have either meaning, but I would read it as leaving again after he came back. If the other meaning had been intended, the writer should have written: I again decided to leave.

I see it the same way as Mike does. If somebody has decided to leave again I would assume he had left before.
