Can you please help me with the the translation of "charge slip" ?

Are you looking for the meaning of the phrase? When you use a credit card, it creates a charge slip: a piece of paper on which the details of the transaction appear.

You can also have pro formas for instructing a company or organization to debit your account. See here for an example: THE JOHNS HOPKINS UNIVERSITY
