Open blue eyes
Does "open blue eyes" that someone has (not for the moment, but in general) mean that s/he has wide-open blue eyes? Or maybe these two have different meaning, and for example wide-open are more open than just open eyes?

This is not a standard phrase.
If you say someone has an "open" look, it's more about personality. You feel you can go talk to that person. This would be in contrast to a "guarded" look or a "haunted look."
Perhaps the person was combining that aspect of a description with the actual color of her eyes.

It's just a description of a person. It's from Jane Walsh's "Not like this":
"Father was tall, golden-haired, with a well-shaped mouth and open blue eyes. Though he himself was Lancashire born, his parents had come over from Ireland, and he had a large quota of Irish charm." And so on...

It means that his eyes looked wide open, rather like a baby's eyes look [and they are blue in colour].
