Question formal letter for a proposal changed to home-based work

to whom it may concern

i would like to know, how to write a short but briefly explains it all for a proposal.

actually i am working at the office. but then my boss said, she wants to changed my status from working at office to home-based work as i am moving to other town.

my works is just data entry clerk, enter the data in the computer using company web based system. the data will be given once a week. every one month, i will send them a report.

i don't want they deduct my salary.
same salary but home based work.
