Default Does this need a comma before as needed?

You do need a comma here as two discrete actions are identified.

As has been commented, these seem to be notations from [maybe] ward rounds or medical consultations, so are expressed almost in shorthand. In these cases, normal grammar rules are at least bent or altogether ignored.
