Exclamation Source and Sentence Structure??

Hi would like to know if the text below me is written correctly.....things i would like to know

A. 1st para, if it is correct method of writing the source.
B. Table, correct method of writing the source below the table.
C. Last Para, Sentence structure if correct or not.

Cost Involved in Severity Claims

These severity statistics reflect the claims experience of 13 professional liability insurance companies that together insure 105,344 dentists, of whom, 78% are general practitioners (Risk Management Survey 4).

Less than $10,000 57.2%
$10,000 - $24,999 16.6%
$25,000 - $49,999 11.8%
$50,000 - $99,999 7.6%
$100,000 - $249,000 5.0%
$250,000 - $499,999 1.2%
$500,000 - $999,999 .5%
$1 million or more .1%

According to the percentages from the above table1.3, less than 2% of the claims involve an amount greater than $250,000, on the other hand over 55% of the claims are for the amount less than $10,000. Over 40% of the claims lie between the range of $10,000 to $249,000; and according to the ADA survey, the statistics suggest that the cost involved is most claims are either for minor injuries or the need for corrective treatments.

According to the percentages from the above table1.3,
The book from which this table is taken apparently has a number of tables, this one being Table 1.3. However, I assume this is the sole table that you are extracting. Hence, it is fine to just refer to it within your submission as 'the above table'



From the above table, it is apparent that less than 2% of (the-omit) claims involve amounts greater than $250,000, whilst over 55% of (the-omit) claims are for less than $10,000. Over 40% of claims fall in the range (of-omit) $10,000 to $249,000; and according to the ADA (survey, the-omit) statistics, most claims are either for minor injuries or the need for corrective treatments.
