It means that someone feels really bad, either physically ill or bad as in guilty, perhaps for having done something not so nice to another person.

Also, "feel shitty".

One more thing to add: it is not considered polite. Among your friends it would be fine, but avoid using it with people you don't know well, especially in formal situations: it can cause offence.
