We would not normally use a title in something of this kind. The normal way would be:

This certificate is awarded to John Doe of Class V...

He was placed first. OR He achieved first place.

"praiseworthy" is one word.
Otherwise the wording seems good.

If it is an informal announcement, then slangy adjectives are acceptable.
