5-It was the first time to go there. (the subject of "to go" is supposed to be furnished by the context).

1- yes, though many would favour the past perfect
2-yes, but not very naturalto me
5-use a possessive adjective to make this clearer
I think 4 is acceptable in AE. If I am not mistaken, I've heard it.
I do agree with you on 4 and 6.
