Default "not just big noting it"

A: He gave me a penicillin.
B: Well, if he's not just big noting it, you'd better see if you can get enough for two.

Can someone please tell me the meaning of this (the bold part)?
    Thanks a lot!

Both sentences are ungrammatical, and the second does not make sense. I would say they should be:

    A: He gave me penicillin.

B: Well, if he's not meticulous about recording it, you'd better see if you can get enough for two.
