Do you say "raise your hand"? or "raise your hands"?

In what context do you want to use this phrase? I think it depends on how many hands do you mean. If one, raise your hand would be ok. But if you mean both hands like during the PT in school or as a punishment etc., raise your hands is fine.

There is also another relevant meaning in the classroom context where a teacher is asking, for instance, how many of the students had completed an assignment.

How would the question be out then? In my opinion, it should still be 'raise your hand' even though the question may have many respondants hands being raised.

But I may be just complicating things! Carry on, Dawood.
