Having a _____ (real, realistic) picture of oneself is the key to everything.

In this sentence, "realistic" is the choice BECAUSE OF THE MEANING OF THE SENTENCE. Realistic picture of oneself means to look at yourself with accurate judgement. Here, it is an adjective modifying picture.

But people say, "Get real" to each other all the time.
