Default perfect tense+past tense

Which of these questions are correct:

1-Have you ever asked me to do something for you which I didn't do?
2-Have you ever asked me to do something for you and I didn't do it?
3-Have you ever asked me to do something for you and I haven't done it?
4-Has it ever happened that you asked me to something for you and I didn't do it?



Which of the following are acceptable replies in that context:

5-Yes, I have asked you to do something which you didn't do.
6-Yes, I have asked you to do something and you didn't do it.
7-Yes, I have asked you to do something and you haven't done it.
8-Yes, it has happened that I asked you to do something and you didn't do it.
