Nothing in that sentence addresses, in a factual sense, whether the water is or is unfit for human consumption. "The people who say it's unhealthy" may know or they may not know.
And "unhealthy" can have other meanings that have no connection to its purity as potable water. For example,"the people" might believe that the addition of fluoride or chlorine makes it unhealthy.
