Default giving feedback to essays

When you give feedback to student's essays, what do you usually say when you can't understand what the writer is talking about or when his supporing sentneces does not support the topic sentence or if the words are not approopriate????

If it is very blatant = "I do not understand what you are trying to say. Please explain."

If possible I try to make sense of the question and then to answer it.
