Please tell me which one is correct and which one not, in term of grammar.

~I haven't actually go to school for the past 3 weeks.
~I haven't actually went to school for the past 3 weeks.
~I haven't actually go to school for the passing 3 weeks.
~I haven't actually went to school for the passing 3 weeks.

really.. confused.. and thank you for your time.
