The safest thing to do would be to enquire of the person how he (or she) would like to be credited.
For example: Dear __________ We would like to post your comments regarding our product on our website. May we have permission to do so? Also, please inform us how you would like your name to be posted. Yours truly, ____________
This will extract two pieces of information. The first will be whether or not the person is comfortable with his opinions being posted at all. The second will be the way the person would prefer to be identified.
I will use my mother as an example (without her actual name). She is a single elderly woman living alone. If she were to comment, she would allow the comment to be posted, however she would not want to be identified as single, or as a woman. Thus, she would reply to the above this way:
Dear Company,
You are welcome to post my comments on your web site. I do not wish to be identified. Please post me as "anonymous."
OR
You are welcome to post my comments. Please identify me only by initials, as I do not wish to reveal my gender or full name.
Yours truly,
A. B. Client
Hope this helps.
