'Please send me a new chequebook' [use whichever spelling suits your country - "-que-" in the UK, "-ck-" in some other places]. Keep it simple. If you bank online (most banks in the UK offer this facility - maybe all do) you'll probably find a link named something like 'Order Stationery', which means you don't have to worry about the wording.
