'Please send me a new chequebook' [use whichever spelling suits your country - "-que-" in the UK, "-ck-" in some other places]. Keep it simple. Give your full name and address at the top of the letter, and the name of the account and the account number as the subject (centred, after the greeting, in a paper letter, but just make it the "Subject:" in an e-mail).

If you bank online (most banks in the UK offer this facility - maybe all do) you'll probably find a link named something like 'Order Stationery', which means you don't have to worry about the wording.

PS: Every bank I've ever used (5 or 6, I guess) has sent a replacement book automatically after I've used - say - 25 of a 30-cheque book. So after you've got the first one, there's nothing to do (unless you write cheques too quickly for the automatic replacement to arrive!)
