How to improve my communication skills

I don't think you're English is as poor as you seem it is, Ravi.

If you wish to talk with others in English, then you must not take time and do it now. Speak to people in English as often as you can. You will soon find out areas you need to brush up on and also ways to improve them.

Spend time here in the forum. If you have questions about English, post them here. Post everything that comes to your mind, whether they might be difficult expressions, forgotten words, or grammar issues, we have lots of teachers and learners to help you out.
