The two most likely grammatical interpretations are;
- distributed on a real 'per patient' basis (that is 50 times the per-patient payment for a hospital with 50 patients, rather than - say - 1000 times that payment for a district)
or
- distributed in dollars rather than in local currency
But I'm afraid what the writer really means is 'distributed as follows:' or 'distributed in this way:'; it's an abuse of 'as such', but bad things happen!
