Default Questions about "that" and "have/had"

Hi all,

Which of the following are correct? I am confused when do I need to use "that" and "have/had" (as present/past perfect tense)

1. The form that I sent to you is not formatted correctly.
2. The form that I have (or had) sent to you is not formatted correctly.
3. The form I sent to you is not formatted correctly.
4. The form I have (or had) sent to you is not formatted correctly.

Default Re: Questions about "that" and "have/had"

1. The form that I sent to you is not formatted correctly. Herewith the correct form, and I apologize for any inconvenience.

Past tense, since this is a completed, finished action in the past.

3. The form I sent to you is not formatted correctly.
This is also correct, but slightly less formal. In a formal letter, (1) is preferable.
