Kindly correct the following sentences
Sub: Leave Extension
Dear Sir,
This refers to the telephone call received from Mr. X regarding the above subject.
He had to re-join the work today, but to vain he had been admitted in the hospital due to high blood sugar and other discomforts. Now his doctor advised him to take another two week for bed rest, so he requested us to grant him an additional leave starting from 22.06.08 to 06.07.08
Under this circumstances, I would request you to look into this matter and kindly do the needful.
Also, note that he will submit the medical certificate upon his arrival.

Under this circumstances, I would request you to look into this matter and (kindly) do the needful.
Please note that he will submit the medical certificate (upon)on his return from leave.(arrival.)
Delete the words in the brackets.
