Default "how long" and "how long time"

a) "he asked me how long I had known her"

b) "he asked me how long time I had known her"

c) "he asked me how long I knew her"

d) "he asked me how long I have/had been knowing her"

please, is any of these sentences correct?
how would a native say that, please?
is "how long" and "how long time" both possible?

a). is correct.
How long time is not possible.

#c is not grammatical and also is unlikely to be used in speech, even colloquially.

I agree, note that you did not stop to know her. You still know her.
How long you have known her (up to now)?
I asked how long you had known her?
