Please tell me which of the following sentences are correct:

i) I request you kindly to grant me leave for one day.
ii) I request you to kindly grant me leave for one day.

=>I request that you kindly grant me leave for one day.

ii) I request you to kindly grant me leave for one day. <split infinitive; but, then again, English isn't Latin. >

Or "I kindly request you grant me leave for one day" forget the 'to' form.
