One problem is the 'comma splice' - joining two sentences with a comma - which you do with your first comma.

One solution is:
Our QC team colleagues are being moved from 5F to 3F: please help to update their phone numbers, as per these details : ...

You could also write:
'... from 5F to 3F. Please help ...'
'... update their phone numbers, as follows: '
