(not a professional teacher) That phrase would not sound natural to most native speakers. There are some phrases using "look" that convey the idea you're after. You could say "If you're in my town, please look me up". A slightly different, but related, phrase is "look in on", meaning "to visit to make a check on". "If you're going to Timbuktu, please look in on my mother, and see how she is".
