Default In a letter to a Judge

Please help. I am drafting a letter to a Judge of the Circuit Court whose name has a "the third" as in: The Honorable James C. Smith, III

I have addressed the letter to The Honorable James C. Smith, III , however how do I write the salutation:
Dear Judge James C. Smith, III or
Dear Judge Smith, III or
Dear Judge Smith

No comma between the name and "III".

The salutation is simply Dear Judge Smith.
