Hi,
In Italy each pupil has a personal booklet containing forms which parents are requested to fill in when their children are absent from school.
In these forms parents are asked to indicate the date of the absence and the reason. This booklet is mostly used to excuse absences, but
teachers can also use it for written communications to the parents (good or bad marks, bad behaviour).
How would you call such a booklet?
1) personal record booklet
2) absences booklet
3) booklet for notes
4) booklet for absences and notices
5)Any other idea?

It's hard to take ideas that are used in one place, but alien to another, and say what we would call it.
It sounds like something I might call a "parent-teacher memo book," since it's used to send memos from parent to teacher or vice versa. But we don't have such a thing - my child has a folder - things from the teacher to the parent go into that one folder.

Actually you also keep the notes. the forms used to explain the absence are sheets divided into two parts. both contain the same elements (date, explanation, signature of the parents and signature of the teacher who collects the form in the first hour of lesson each day). When a pupil is absent the parents fill in both parts; the teacher signs both and puts a form in the register; the other part remains in the booklet.
