The only exception is when 'know' doesn't refer to knowledge, and the question is rhetorical:
'Do you know what is bugging me? He must have known before...otherwise he wouldn't...'. This is easy to remember: 'bugging me' isn't a noun. When you're asking about a noun, invert the 'direct question' order.
