How about:

Patron was called in regards to an overdue library notice for book "X". He said he left the book "X" in a box while he was living out of the town in "x". He was reminded of the library's maximum overdue fines and replacement cost policy for unreturned books.

(You don't have to worry if the book is lost or not. The library's concern is that the book has not been returned!)

Patron called in response to a (library) overdue book notice regarding book 'X'. He said he left the book in a box while he was living out of town in 'x'. The patron was informed of the maximum overdue fines and the replacement cost if the book has been lost.

I hope this is helpful,

Petra
