Dear teachers,
______________he gets an operation, the boy may lose his eharing froever.
Can I fill in the blanks with the word 'If' ?
Looking forward to hearing from you.
Thank you in advance.

If you put 'if' it would mean that the boy would lose his hearing as a result of the operation, I doubt that that is what is intended: I would suggest something else, what do you think?
Not a teacher,
I guess this sentence means:
Unless he gets an operation, the boy may lose his hearing forever.
I would suggest 'unless' as inorderto has said in his post.
Sorry: I should not have assumed, I should have written 'his or her'.
