comma and grammar questions...

Hi, I'm a student, not a teacher and was wondering if someone could help me with some comma problems I'm having, as well as some grammar questions. They are as following:

1) Quite symptomatic for depression with significant anhedonia, despite Prozac.

Is there a comma before "despite" here or not?

2) Her father died of kidney disease, and her mother of Alzheimer’s.

Is there a comma before "and" here or not?

3) Tinel’s positive, right > left, with associated Phalen’s.

Is there a comma before "right" and after "left" here or not?

4) Bilateral carpal tunnel syndrome, right > left.

Is there a comma before "right" here or not?

5) It might be helpful to change from Didronel to alendronate 70 mg weekly, as this agent is considerably more potent.

Is there a comma before "as" here or not?

6) Depression – persistently symptomatic, despite Prozac.

Is there a comma before "despite" here or not?

7) EMG and nerve conduction studies, both extremities, and one lower extremity.

Are there commas before "both" and after "extremities" here or not?

8) Increase calcium supplement to 1,500 mg-1,800 mg/day, given in divided doses after eating.

Is there a comma before "given" here or not?


Okay, those are my comma questions. Now here are my grammar questions. The following sentence:

a) Obtain additional info from Dr. Smith including the previous X-ray studies.

If you are transcribing a file and you hear the dictator say "info", is it grammatically correct to shorten "info" or are you required grammatically to spell it out?

b) The numbness and dysesthesias are worse in the right 1-3 digits.

Is dysesthesias supposed to have an "s" or not? I've seen it 'most' times without an "s"; however, when i hear the dictator say the word, it sounds like he says it with an "s". dysesthesia vs. dysesthesias

c) Heart disease – followed by Dr. Smith for, what sounds like, valvular heart disease and bradyarrhythmias.

Is the word "bradyarrhythmias" supposed to have an "s" or not? Same situation here, I've seen Heart disease – followed by Dr. Smith for what sounds like valvular heart disease and bradyarrhythmia without an "s" before but it sounds like he says "s" at the end. bradyarrhythmias vs. bradyarrhythmia. Also, is there supposed to be a comma before "what" and after "like" here or not?


Okay, that's all I got for now! Any help would be much appreciated! Thanks in advance if there is anyone who knows about these problem areas..

You should have a medical terminology book or dictionary for the spellings.

I figured out most of these I think. Still confused on whether I should have commas used on 3) though.

Tinel's positive, right > left, with associated Phalen's.

OR

Tinel's positive right > left with associated Phalen's.
