I do hereby wish to inform you that she is a thallassemia ( intermediate ) child and needs blood transfusion to maintain her haemoglobin level at an interval of 30 to 40 days . I reuest you to consider her for special leaves (if required ) for her medical checkups .
I shall be highly obliged for your kind co-operation in this matter .
