I agree. a or the or even this has been omitted, probably to save space on the sign.

1. Security alarm will sound if (a/the) door is opened. (Can 'door' also be an uncountable noun here because you don't know if it is one door or two doors?

I'm not sure I understand what you mean by door 'can be uncountable'. It's a count noun, so it's always countable, or rather it requires a determiner--unless, that is, it's a sign on a door, and then small words are often omitted. In our example, the most likely case, given that the sign is on a specific door, is:

(The/Our) security alarm will sound if (this) door is opened.
